Nursing Diagnosisis essential for choosing the right nursing interventions in order to achieve the expected outcomesfor which a nurse is accountable. Nursing diagnoses are used to determine the appropriate plan of care for the patient, driving patient outcomes and interventions. Nursing diagnoses also provide a standard language for use in the Electronic Health Record (EHR), enabling clear communication among care team members and the collection of data for continuous improvement in patient care. But it is not easy for nurses to give a clinical judgment about a human response to health conditions and life processes.
